Entecavir

Entecavir INN, disingkat ETV, adalah obat antivirus mulut yang digunakan untuk penanganan infeksi virus hepatitis B (HBV). Entecavir adalah pencegah transkriptase terbalik. Obat ini mencegah virus hepatitis B menggandakan diri dan mengurangi jumlah virus di dalam tubuh.

Mekanisme sunting

Entecavir adalah analog nukleosid, atau lebih rincinya, Entecavir adalah analog deoksiguanosin yang menghambat transkripsi terbalik, replikasi DNA, dan transkripsi dalam proses replikasi virus.

Kegunaan sunting

Entecavir digunakan untuk menangani infeksi hepatitis B kronis yang diderita dewasa dan anak berusia 2 tahun atau lebih dengan replikasi virus aktif dan bukti penyakit aktif. Obat ini juga dipakai untuk mencegah reinfeksi HBV setelah transplantasi hati dan menangani penderita HIV yang terinfeksi HBV. Entecavir tidak begitu aktif melawan HIV, namun tidak disarankan untuk penderita HIV-HBV tanpa regimen anti-HIV yang bekerja penuh karena obat ini resisten terhadap lamivudin dan emtrisitabin dalam HIV.

Pemakaian sunting

Entecavir dikonsumsi saat perut kosong, sedikitnya 2 jam sebelum atau sesudah makan.

Efek samping sunting

Efek samping yang paling umum dari entecavir adalah sakit kepala, lelah, pusing, dan mual. Efek samping lainnya adalah diare, dispepsia, muntah, somnolensi, dan insomnia. Ketaknormalan klinis yang diakibatkan oleh perawatan menggunakan entecavis meliputi naiknya jumlah alanin transaminase (ALT) dan hematuria. Pasien disarankan menjalani pengawasan rutin fungsi hepatik dan hematologi.

Uji klinis sunting

Keampuhan klinis entecavir telah diteliti dalam berbagai pengujian acak, buta, dan multipusat. Entecavir mulut merupakan penanganan yang efektif dan lazim.

Bristol-Myers Squibb adalah pihak pertama yang memegang paten Baraclude®, nama merek entecavir di Amerika Serikat dan Kanada. Paten Baraclude® kedaluwarsa tahun 2015. Pada tanggal 26 Agustus 2014, Teva Pharmaceuticals USA mendapat persetujuan FDA untuk memproduksi tablet generik Baraclude® 0,5 mg dan 1 mg; Hetero Labs mendapat persetujuan tanggal 21 Agustus 2015; dan Aurobindo Pharma tanggal 26 Agustus 2015.
