Gelar resminya adalah Somdet Phra Theppharatratsuda Chao Fa Maha Chakri Sirindhorn Ratthasimakhunakonpiyachat Sayamborommaratchakumari (สมเด็จพระเทพรัตนราชสุดา เจ้าฟ้ามหาจักรีสิรินธร รัฐสีมาคุณากรปิยชาติ สยามบรมราชกุมารี), yang diberikan kepadanya pada 5 Desember 1977. Warga Thailand biasa menyebutnya dengan gelar Phra Thep, yang berarti Putri Bidadari.

Kedudukannya dalam pewaris takhta Thailand sejajar dengan gelar kakaknya Pangeran Mahkota, Maha Vajiralongkorn. Hal ini disebabkan oleh perubahan konstitusi Thailand pada tahun 1974 yang memperbolehkan pewaris takhta dari seorang wanita, dan akhirnya Sirindhorn berhak atas kekuasaan di Thailand.
