Centineo dilahirkan diMiami, Florida, putra dari Kellee Janel dan Gregory Vincent Centineo, seorang pengusaha eksekutif yang memproduksi film animasi Legends of Oz: Dorothy's Return (2013). Ia keturunan Italia dan Jerman.

Centineo dibesarkan di Boynton Beach, Florida, bersama kakak perempuannya, Taylor. Ia belajar di BAK Middle School of the Arts untuk pendidikan dasarnya, dan kemudian Boca Raton Community High School untuk kelas sembilan dan sepuluh, di mana ia bermain sepak bola. Pada tahun 2012, ia meninggalkan Boca Raton untuk pindah ke Los Angeles.
